Introduction

A diagnosis of prostate cancer often causes concern and raises many questions. When it is localized (that is, when it remains confined to the prostate gland, without spreading to the lymph nodes or metastases), curative treatment options exist.

The goal is to confidently choose, with a good benefit/risk balance, the strategy best suited to your situation: to heal, to control, while preserving your quality of life.

What is meant by “localized prostate cancer”?

  • The PSA (prostate-specific antigen) level
  • The Gleason score (degree of cell aggressiveness)
  • Stage T (TNM)
  • The tumor volume, its location
  • Your general health, age, comorbidities, personal expectations

Treatment options

  • Indicated in cases of low-risk cancer (small tumor, low PSA, low aggressiveness) and if your life expectancy is not far off.
  • It consists of monitoring the cancer without intervening immediately, using regular checks: PSA, rectal examination, MRI, repeated biopsies if necessary.
  • If the cancer shows signs of progression, curative treatment is then considered.
  • Complete removal of the prostate (and often the seminal vesicles), sometimes combined with lymph node dissection.
  • It can be performed via open, laparoscopic or robot-assisted approach (with benefits in terms of surgical precision).
  • It offers a radical, potentially curative option, but with risks (incontinence, erectile dysfunction).
  • Cancer is treated with targeted ionizing radiation to destroy cancer cells.
  • It can be used alone or in combination with hormone therapy depending on the risk of cancer.
  • It is spread over several weeks.
  • Possible side effects: bladder irritation, urinary problems, digestive issues, sometimes impacting sexual function.
  • Radioactive implants (seeds) are placed directly into the prostate to deliver radiotherapy internally.
  • This option is valid depending on the tumor volume, aggressiveness and architecture of the prostate.
  • For certain well-localized tumors of low to medium risk, focal techniques may be considered (HIFU, cryotherapy, electroporation, micro-treatments) in order to treat the cancerous area while sparing healthy tissues.
  • These techniques potentially reduce side effects (incontinence, erectile dysfunction) but are not suitable for all cases.
  • In certain situations (intermediate or high risk), hormone therapy (blocking the effects of testosterone) is added to local treatment to improve the chances of control.
  • It can be used before, during or after radiotherapy depending on the case.

FAQ – Frequently Asked Questions

What are the options for treating localized prostate cancer?

Three main strategies exist: active surveillance for low-grade cancers, radical prostatectomy (most often robot-assisted), and radiotherapy, possibly combined with a short course of hormone therapy. The choice is made in multidisciplinary consultation.

How to choose between surgery and radiotherapy?

Both offer excellent cancer control results. The choice takes into account age, tumor characteristics, pre-existing urinary problems, the side effects specific to each treatment, and your preferences after receiving full information.

What is active surveillance?

For low-risk cancers, the disease is monitored with regular PSA tests, MRIs, and follow-up biopsies, without immediate treatment. Curative treatment is only initiated if the disease shows signs of progression—thus sparing many patients the side effects of treatment.

What are the chances of recovery?

For localized cancer, 10-year survival rates exceed 95% regardless of the curative treatment chosen. Follow-up then relies on simple, regular PSA testing.
